Law On Protectıon Of Personal Data Applıcatıon Form

As set forth and stated by the Law on Protection of Personal Data Nr. 6698 (“Law On The Use Of Personal Data”), the application to be filed to the data supervisor by the real entities with the personal data processed (“Relevant Person”) for the purpose of exercising the rights thereof on their personal data can be delivered in written or through other methods to be determined by the Board of Protection of Personal Data.
 
Without prejudice to other methods to be determined by the Board, the application methods determined by Zeplin considering the criteria of “being written” of our positive regulation for the relevant persons are stated as below. The relevant persons are entitled to file application by using any of the methods stated herein by taking this form as basis. In accordance with this, the Relevant Person may file application by
 
- sending an e-mail message to res@zeplincar.com with secure electronic signature or
- personally applying to Kore Şehitleri Caddesi No:38/8 Zincirlikuyu/İstanbul - with wet signature to be appended on this form or
- public notary

 
The applications delivered to Zeplin by using any of the above-cited methods shall be responded free of charge as a rule within no later than 30 days and in the shortest time possible based on the nature of the request and this response shall be delivered to the Relevant Person in written or on electronic environment. In the event that the request requires further cost, the fees to be determined by the Board shall be charged to the Relevant Person.
